Children and Young People’s Multi-Agency Navigation Hub
The Children & Young People’s Multi-Agency Navigation Hub brings together Education, Health, Voluntary Sector and Early Help services to collectively provide advice, assessment, or intervention through a streamlined point of contact. Gloucestershire’s County Council & Integrated Care Board are pleased to announce that the Children and Young People’s Multi-Agency Navigation Hub is now the single point of contact for both Mental Health and Education Inclusion related referrals from ALL Gloucester City Schools.
The Navigation Hub integrates CAMHS, Young Minds Matter, Education Inclusion, TiC+, Young Gloucestershire, School Nursing, Early Help, Music Works and Play Gloucestershire with hopes to widen the scope in the future to include additional children and family services.
Our long-term ambition is to transform access to our partnership of agencies supporting children and young people across the whole county, starting with Gloucester City. We are pleased to announce that the Children and Young People’s Multi-Agency Navigation Hub is now the single point of contact for both Mental Health and Education Inclusion related referrals from ALL Gloucester City Schools.
All support requests for Mental Health and Education Inclusion Services from Gloucester City Schools must be directed though the Navigation Hub from 1st February 2024.
How to request support from the Children & Young People’s Multi-Agency Navigation Hub from 1st February 2024
- Identification of a child or young person who is struggling with their mental health or experiencing barriers to their education.
- Discussion with the child or young person (and family if appropriate) to understand what help they are looking for so support can be tailored to their individual needs.
- School complete the Navigation Hub Involvement Request Form which facilitates the different information required by all agencies. This means you only have to complete a request for support once. It is important to ensure that all the mandatory information is completed including any existing support plans in place. This allows us to respond as quickly as possible and ensure the most appropriate support is suggested. Incomplete requests for support may be returned to the referrer for completion.
- The request for support will get discussed at the next available multi-agency panel; this group meets twice weekly. The panel review the information available to them and make an informed decision about the most appropriate support to best meet the needs of the young person. The agency/agencies most appropriate accepts the request for support.
- The Navigation Hub communicates the outcome of the panel to the School following the meeting.
The Children & Young People’s Multi-Agency Navigation Hub will be active Monday – Friday during office hours 9:00 – 17:00. Further information can be accessed from the webpage.
